Working together to restore function so you can regain independence.

Chestnut Hill Hospital’s Rehabilitation Services program can help people who have experienced disabling diseases or injuries to regain control of their lives by restoring movement and function.

Physical, occupational and speech therapists will create individualized therapy plans that consider the unique needs, abilities and objectives of each patient. Together with your physician, we will work to help restore you to the highest level of function possible.

Our rehab staff is diversely trained and experienced in the various stages of injury and illness. They treat patients from the most acute stages at the hospital bedside to full recovery in our outpatient setting.

Plan of Care

Since no two people or injuries are exactly alike, therapy is customized to make meaningful, practical and sustained changes in function.

An assessment, including a medical history, tests and measurements, will allow your therapist to formulate a Plan of Care for your recovery.

You and a therapist will establish goals focused on maximizing functional independence, preventing further movement/function problems and maintaining health.  To help achieve goals, Chestnut Hill Hospital offers a variety of skilled therapy interventions including:

  • Education: understanding your condition, treatment and how to care for yourself.
  • Functional Training: skills aimed at obtaining efficiency in tasks required for everyday living.
  • Prescription and Application of Devices or Equipment: to aid or support functional mobility.
  • Therapeutic Exercise: including written instruction to enable continuation of therapy at home.
  • Interventions and Modalities: to improve motion, promote healing and reduce pain.
